Scope of the website
We provide information about the clinic and services, along with a form that sends messages to the clinic, free of charge. The website does not take payments, contain a shop or independently confirm bookings. Use of these functions begins when you open the relevant page; you can stop by closing the view.
Bookings and prices
Bookings are made through Booksy or by arrangement with the clinic. Before booking, check the price, scope, duration and conditions for changing or cancelling an appointment through your chosen channel. Preparing or sending an enquiry does not confirm an appointment or order a treatment. The website price list is for information; the clinic can confirm whether a particular entry is current.
Treatment information
Descriptions do not constitute a diagnosis or individual medical recommendation. The choice of service, indications, contraindications and expected results require an appropriate consultation. Photographs and illustrations are for presentation purposes; they do not document treatment outcomes or guarantee results.
Requirements and use of the form
You need an up-to-date browser with an internet connection; interactive features require JavaScript. The “Send message” button forwards the form directly to the clinic’s email service. A success message means acceptance for sending, not confirmation of a booking or that the message has been read. If sending is unavailable, use the clinic’s email address or telephone number. We apply limits to protect against spam. Do not enter medical data, a PESEL number or unlawful content, or attempt to disrupt the website.

Report issues with the website to kontakt@slimsmileclinic.pl or in writing at the clinic address. Describe the situation, the date it occurred and how to contact you. We will reply within 14 days of receiving the enquiry. These terms do not limit consumers’ statutory rights.
